ABSTRACT

O presente estudo tem como objetivo relatar o caso de uma paciente com malformação arteriovenosa uterina, efetivamente tratada com embolização seletiva e com fertilidade preservada. A malformação arteriovenosa uterina é uma alteração vascular rara até então pouco descrita na literatura. A paciente do sexo feminino apresentou quadro de sangramento uterino anormal, com início 30 dias após um abortamento, sem realização de curetagem, de uma gestação resultante de fertilização in vitro. Foram, então, realizados exames de imagem, que levaram ao diagnóstico de malformação arteriovenosa uterina. O tratamento de escolha foi a embolização arterial seletiva, com resolução do caso. Após sete meses, nova fertilização in vitro foi realizada, encontrando-se na 36a semana de gestação. São necessários mais estudos sobre essa malformação a fim de que sejam estabelecidos os métodos mais eficazes para o manejo de casos futuros, especialmente quando há desejo de gestar.


The present study aims to report the case of a patient with uterine arteriovenous malformation, effectively treated with selective embolization and with preserved fertility. Uterine arteriovenous malformation is a rare vascular disorder that has so far been rarely described in the literature. Female patient presented with abnormal uterine bleeding, starting 30 days after an abortion without subsequent curettage, of a pregnancy resulting from in vitro fertilization. Imaging tests were then performed that led to the diagnosis of uterine arteriovenous malformation. The treatment of choice was selective arterial embolization, with successful results. After seven months, a new in vitro fertilization was performed, being in the 36th week of pregnancy. Further studies on this pathology are needed in order to establish the most effective methods for the management of future cases, especially when there is a desire to become pregnant.

ABSTRACT

Abstract This study aimed to assess the influence of the file format on the image quality parameters (image noise, brightness, and uniformity) of periapical radiographs acquired with different digital systems. Radiographic images of an acrylic phantom were acquired with two digital systems - Digora Toto and Express, and exported into five different file formats - TIFF, BMP, DICOM, PNG, and JPEG. Image noise, image brightness (mean of gray values), and image uniformity (standard deviation of gray values) were evaluated in all images. A two-way analysis of variance with Tukey's test as a post-hoc test was used to compare the results, considering the file formats and radiographic systems as the studied factors. A significance level of 5% was adopted for all analyses. The DICOM image file format presented lower image noise, higher brightness (higher mean gray values), and greater image uniformity (p<0.001) than the other file formats, which did not differ from each other for both digital radiography systems (p>0.05). The Express system revealed lower image noise and greater image uniformity than the Digora Toto system regardless of the image file format (p<0.001). Moreover, the Express showed higher brightness than the Digora Toto for all image file formats (p<0.001), except for the DICOM image file format, which did not significantly differ between the digital radiography systems tested (p>0.05). The DICOM image file format showed lower image noise, higher brightness, and greater image uniformity than the other file formats (TIFF, BMP, PNG, and JPEG) in both digital radiography systems tested.


Resumo Este estudo teve como objetivo avaliar a influência do formato do arquivo nos parâmetros de qualidade de imagem (ruído de imagem, brilho e uniformidade) de radiografias periapicais adquiridas com diferentes sistemas digitais. As imagens radiográficas de um fantoma de acrílico foram adquiridas com dois sistemas digitais - Digora Toto e Express, e exportadas em cinco formatos de arquivo diferentes - TIFF, BMP, DICOM, PNG e JPEG. O ruído da imagem, o brilho da imagem (média dos valores de cinza) e a uniformidade da imagem (desvio padrão dos valores de cinza) foram avaliados em todas as imagens. A análise de variância (ANOVA) bidirecional com o teste post-hoc de Tukey foi aplicada para comparar os resultados, considerando os formatos de arquivo e os sistemas radiográficos como os fatores estudados. Um nível de significância de 5% foi adotado para todas as análises. O formato de arquivo de imagem DICOM apresentou menor ruído de imagem, maior brilho (maiores valores médios de cinza) e maior uniformidade de imagem (p<0,001) do que os outros formatos de arquivo, que não diferiram entre si para ambos os sistemas de radiografia digital (p>0,05). O sistema Express apresentou menor ruído de imagem e maior uniformidade de imagem que o sistema Digora Toto, independentemente do formato do arquivo de imagem (p<0,001). Além disso, o Express mostrou maior brilho do que o Digora Toto para todos os formatos de arquivo de imagem (p<0,001), exceto para o formato de arquivo de imagem DICOM, que não diferiu significativamente entre os sistemas de radiografia digital testados (p>0,05). O formato de arquivo de imagem DICOM mostrou menor ruído de imagem, maior brilho e maior uniformidade de imagem do que os outros formatos de arquivo (TIFF, BMP, PNG e JPEG) em ambos os sistemas de radiografia digital testados.

ABSTRACT Objective: To evaluate the clinical profile, pain improvement, and the need for surgical interventions in patients undergoing transforaminal block with the use of corticosteroids and anesthetics. Methods: This is a prospective, randomized, double-blind study with 45 patients with unilateral radicular pain in their lower limbs and a single-segment lumbar disc herniation diagnosis. In the intervention group, transforaminal blocks with bupivacaine, dexamethasone, and clonidine were applied and in the control group, distilled water and bupivacaine. The Oswestry questionnaire was applied. Results: We included 24 female (53.4%) and 21 male patients (46.6%). Of those with an occupation, 85.71% (n = 30) were relieved from their duties due to their illness and 14.29% (n = 5) continued to work with limitations. Those who underwent transforaminal block with an injection of corticosteroids, clonidine, and anesthetics showed immediate relief. However, such effect failed to alleviate patients' symptoms after three weeks. We observed that 52% of patients showed varying degrees of improvement. The control group experienced mild pain relief after one week, which also failed to last after three weeks. Moreover, 50% of patients improved in varying degrees. Conclusion: Further studies with larger samples, new epidemiological data, and longer follow-ups are necessary to validate our hypotheses. Level of Evidence II, Prospective Study.


RESUMO Objetivo: Avaliar o perfil clínico, a melhora da dor e a necessidade de intervenções cirúrgicas em pacientes submetidos ao bloqueio transforaminal com uso de corticosteroides e anestésicos. Métodos: Estudo prospectivo, randomizado e duplo-cego realizado com 45 pacientes com dor radicular unilateral em membros inferiores e diagnóstico de hérnia discal lombar em um único segmento. No grupo intervenção, os bloqueios transforaminais foram feitos com bupivacaína, dexametasona e clonidina; no controle, água destilada e bupivacaína. Foi aplicado questionário de Oswestry. Resultados: A amostra foi composta de 24 mulheres (53,4%) e 21 homens (46,6%). Dos pacientes com ocupação, 85,71% (n = 30) estavam afastados de suas funções devido à doença e 14,29% (n = 5) continuavam a trabalhar com limitações. Os que foram submetidos ao bloqueio transforaminal com injeção de corticoide, clonidina e anestésico apresentaram alívio imediato. Após três semanas, contudo, o efeito não perdurou de forma tão satisfatória, e 52% dos pacientes apresentaram melhora em graus variados. No grupo controle, houve discreto alívio álgico após uma semana, que não perdurou de forma satisfatória após três semanas, com 50% dos pacientes evoluindo para melhora em graus variados. Conclusão: Mais estudos com espaço amostral maior, novos dados epidemiológicos e seguimento mais prolongado são necessários para validar as hipóteses aventadas. ABSTRACT

Abstract Hydrocotyle umbellata L., Araliaceae, is a species that is recommended in Ayurvedic medicine for its effects on the central nervous system, such as anxiolytic and memory-stimulant effects. Despite the medicinal potential of this species, its phytopharmaceutical and technological potential to produce standardized extracts has not been investigated. This study analyzes the influence of spray drying parameters on the contents of the chemical markers (total phenolic, total flavonoid, and hibalactone) and the functional properties of H. umbellata extract. The optimization of drying conditions was performed using a central composite design combined with response surface methodology and desirability function approach. The mathematical models fitted to experimental data indicated that all the evaluated drying parameters significantly influenced the chemical contents. The optimal conditions were: inlet temperature of 120 °C, feed flow rate of 4 mL min-1, and colloidal silicon dioxide:maltodextrin ratio of 16%:4%. Under these conditions, the powder samples had spherical particles and desirable physicochemical and functional properties, such as low water activity and moisture content, good product recovery, reconstitution, and flowability. Thus, spray drying might be a promising technique for processing standardized H. umbellata extracts.

ABSTRACT

Purpose: To evaluate antimalarial with or without anticoagulant treatment, in patients with recent SARS-COV-2 infection. Methods: Clinical study carried out at Samuel Libânio Clinic Hospital, University of Vale do Sapucaí, Pouso Alegre-MG. Approved by the Ethics Committee (4.034.077) and registered in the Clinical Trials (NCT04788355). Suspected patients for COVID-19 were included in the emergency room. The groups were: C (control) with 6 patients, A (anticoagulant apixaban) with 9 patients, H (hydroxychloroquine) with 5 patients and HA (hydroxychloroquine and anticoagulant apixaban) with 8 patients. Results: there were no significant differences between groups. The HA group, in which there was an intervention with two drugs, presented a greater number of days with symptoms (p = 0.037) and worse results, when compared to the control: most relevant symptoms, were: cough (p = 0.001), and anosmia / ageusia (p = 0.011) headache (p = 0.001). Conclusion: The present study began when there were doubts about the use of drugs such as Hydroxychloroquine (HCQ) and apixaban (APX). The reduced "n" was defined through bureaucratic and polemic issues independent of the authors' actions. No clinical benefit was associated with HCQ and APX. There was an increase in the number of symptomatic days when HCQ and APX were administered. Despite the limitations, there was no therapeutic indication of the evaluated drugs. [au]


Objetivo: Avaliar antimalárico com ou sem tratamento anticoagulante, em pacientes com infecção recente por SARS-COV-2. Métodos: Estudo clínico realizado no Hospital das Clínicas Samuel Libânio da Universidade do Vale do Sapucaí, Pouso Alegre-MG. Aprovado pelo Comitê de Ética (4.034.077) e registrado nos Ensaios Clínicos (NCT04788355). Pacientes suspeitos de COVID-19 foram incluídos na sala de emergência. Os grupos foram: C (controle) com 6 pacientes, A (anticoagulante apixabana) com 9 pacientes, H (hidroxicloroquina) com 5 pacientes e HA (hidroxicloroquina e anticoagulante apixabana) com 8 pacientes. Resultados: não houve diferenças significativas entre os grupos. O grupo HA, no qual houve intervenção com dois medicamentos, apresentou maior número de dias com sintomas (p = 0,037) e piores resultados, quando comparado ao controle: os sintomas mais relevantes foram: tosse (p = 0,001), e anosmia/ageusia (p = 0,011) cefaléia (p = 0,001). Conclusão: O presente estudo teve início quando havia dúvidas sobre o uso de medicamentos como hidroxicloroquina (HCQ) e apixabana (APX). O "n" reduzido foi definido por meio de questões burocráticas e polêmicas independentes das ações dos autores. Nenhum benefício clínico foi associado com HCQ e APX. Houve um aumento no número de dias sintomáticos quando HCQ e APX foram administrados. Apesar das limitações, não houve indicação terapêutica dos medicamentos avaliados. [au]

ABSTRACT

SUMMARY OBJECTIVE: The objective of this study was to evaluate whether a single measurement of vascular endothelial growth factor could distinguish between intrauterine pregnancy and ectopic pregnancy and to correlate the levels of vascular endothelial growth factor with serum levels of progesterone andβ-human chorionic gonadotropin in each subgroup. METHODS: Ninety patients with a positive human chorionic gonadotropin test and either abdominal pain or vaginal bleeding were selected; pregnancies were singletons, spontaneously conceived, 42-56 days of gestational age. All patients had a transvaginal ultrasound examination and were divided into three subgroups: abnormal intrauterine pregnancy, tubal pregnancy, and normal intrauterine pregnancy. Tubal pregnancies were surgically treated and histologically confirmed. Blood samples were collected for the determination of β-human chorionic gonadotropin, progesterone, and vascular endothelial growth factor and their concentrations were compared in each subgroup. Receiver operating characteristic curve was calculated by comparing the subgroup of tubal pregnancy to the other groups. A Fisher discriminant function analysis was performed. The level of significance was 5%. RESULTS: One-way analysis of variance revealed a significant correlation between the different subgroups and β-human chorionic gonadotropin, progesterone, and vascular endothelial growth factor serum levels (p<0.001). Vascular endothelial growth factor concentration was significantly higher for patients with tubal pregnancy than for other subgroups (p<0.05). β-Human chorionic gonadotropin and progesterone levels were higher in the subgroup with normal intrauterine pregnancies compared with the subgroups with tubal and abnormal intrauterine pregnancies (p<0.05). Serum vascular endothelial growth factor level >188.7 ng/mL predicted tubal pregnancy with 96.7% sensitivity, 95.0% specificity, 90.6% positive predictive value, and 98.3% negative predictive value. CONCLUSIONS: Serum vascular endothelial growth factor could be a marker in discriminating intrauterine pregnancy from tubal pregnancy; its levels are increased in women with ectopic pregnancy compared with women with normal and abnormal intrauterine pregnancies.

Abstract This study aimed to describe the imaging aspects of a concha bullosa discovered incidentally by means of cone-beam computed tomography (CBCT) imaging. A female patient underwent a CBCT exam, and a bilateral extensive pneumatization of the middle concha was verified, presenting a larger dimension of the right side and a deviated septum. The patient reported nasal obstruction and loss of smell. The CBCT for covering the maxillofacial region allowed the discovery of this incidental finding, allowing the patient to be referred for appropriate treatment.


Resumen Este estudio tuvo como objetivo describir los aspectos imaginológicos de una concha bullosa hallada incidentalmente por medio de imágenes de tomografía computarizada de haz cónico (TCHC). Una paciente de sexo femenino fue sometida a examen de TCHC y se verificó una neumatización extensa bilateral de la concha media, presentando una mayor dimensión del lado derecho, asi como desvío del tabique. El paciente refirió obstrucción nasal y pérdida del olfato. La TCHC de la región maxilofacial permitió localizar este hallazgo incidental, permitiendo derivar al paciente para el tratamiento adecuado.

ABSTRACT

This study investigated changes in anxiety and depression symptoms after two exercise programs with adult women attending the Health Academy Program. Based on non-probabilistic sampling, a total of 93 adult women (aged range: 19­77 years) were recruited into two health units, selected to receive exercise programs with continuous (CT; n = 53) or intermittent (IT; n = 40) characteristics. The activ-ities were supervised for six months (twice a week; ~60 min) by a physical education professional. The primary outcome of the study was the assessment of the level of changes of anxiety and depression using the Hospital Anxiety and Depression scale. Physical function was assessed by a 1-mile walk test, handgrip strength, and sit-to-stand test. No significant group-by-time interaction was observed for any of the investigated outcomes. Regarding the comparisons between moments, there were sig-nificant improvements in anxiety (F = 16.52; p = <0.001; ηp2 = 0.15) and depression scores (F = 9.29; p = 0.003; ηp2 = 0.09). Furthermore, there were significant improvements in the one mile walk test (F = 70.36; p = <0.001; ηp2 = 0.44), sit-to-stand test (F = 141.53; p = <0.001; ηp2 = 0.61) and handgrip strength performance (F = 10.12; p = 0.002; ηp2 = 0.10). In conclusion, both exercise programs were equally effective in promoting beneficial changes on anxiety, depression levels, and improved physical function in women attending the Health Academy Program. Therefore, the decision to choose which protocol to use should be based on community preference and practical considerations


Este estudo investigou mudanças nos sintomas de ansiedade e depressão após dois programas de exercícios com mulheres adultas participantes do Programa Academia da Saúde. Com base em amostragem não probabi-lística, um total de 93 mulheres adultas (faixa etária: 19 a 77 anos) foram recrutadas em duas unidades de saúde, selecionadas para receber programas de exercícios físicos com características contínua (CT; n = 53) ou intermitente (IT; n = 40). As atividades foram supervisionadas durante seis meses (duas vezes por semana; ~60 min) por um profissional de educação física. O desfecho primário do estudo foi a avaliação do nível de alterações de ansiedade e depressão por meio da escala Hospitalar de Ansiedade e Depressão. A função física foi avaliada por meio do teste de caminhada de 1 milha, força de preensão manual e teste de sentar e levantar. Nenhuma interação grupo-por-tempo significativa foi observada para qualquer um dos resultados investigados. Em relação às comparações entre os momentos, houve melhora significativa nos escores de ansiedade (F = 16,52; p = <0,001; ηp2 = 0,15) e depressão (F = 9,29; p = 0,003; ηp2 = 0,09). Além disso, houve melhorias significativas no teste de caminhada de 1 milha (F = 70,36; p = <0,001; ηp2 = 0,44), teste de sentar e levantar (F = 141,53; p = <0,001; ηp2 = 0,61) e desempenho de força de preensão manual (F = 10,12; p = 0,002; ηp2 = 0,10). Em conclusão, ambos os programas de exercícios foram igualmente eficazes na promoção de mudanças benéficas nos níveis de ansiedade, depressão e melhora da função física em mulheres participantes do Programa Academia de Saúde. Portanto, a decisão de escolher qual protocolo aplicar deve ser baseada na preferência da comunidade e em considerações práticas

ABSTRACT

SUMMARY OBJECTIVE: The objective of this study was to emphasize the importance of legal and bioethical knowledge in maintaining medical confidentiality, especially in situations when there is a diagnosis of HIV infection. METHODS: A literature review of studies published in the Scientific Electronic Library Online and National Library of Medicine databases was performed. Sixteen studies available in full, online, and free, published between 2010 and 2020, were selected. RESULTS: The studies highlighted that, despite the ethical duty to breach confidentiality for the protection of third parties, many doctors are reluctant to reveal this secret due to the power of stigmatization and social discrimination related to the diagnosis of HIV infection, which affects integrity, counseling, and capability to treat patients. CONCLUSION: HIV diagnosis implies bioethical and legal questions. Respect for medical confidentiality is a matter to be discussed, as there is a need to protect the privacy of the patient, at the same time the responsibility to preserve the health of others.

SUMMARY OBJECTIVE: The objective of this study was to measure the intra- and inter-rater reliability of the quantitative sensory testing for measuring the thermal pain threshold on myofascial trigger points in the upper trapezius muscle of individuals with chronic neck pain. METHODS: Thirty female participants were included, aged between 18 and 45 years and with bilateral myofascial trigger points, active and centrally located in the upper trapezius muscle. Two measurements with quantitative sensory testing were performed by each examiner at an interval of 1 week between them. RESULTS: We observed substantial reliability for the intra-rater analysis (intraclass correlation coefficient ranging between 0.876 and 0.896) and excellent reliability for the inter-rater analysis (intraclass correlation coefficient ranging between 0.917 and 0.954). CONCLUSION: The measurement of the thermal pain threshold on myofascial trigger points in individuals with chronic neck pain has acceptable reliability values, supporting the use of the quantitative sensory testing in the research setting and the clinical environment.

ABSTRACT: This study evaluated patients´ anxiety levels related to dental radiographic examinations in addition to dentists' perception and management. Salivary alpha-amylase (sAA) activity of 47 patients was measured before and after exposure to radiographic examinations to assess their anxiety level and compared using Wilcoxon test. Mann-Whitney test assessed the sAA activity related to sex, type of examination, and previous experience with radiographic examinations. 50 dentists were interviewed using a semi-structured script containing open-ended questions perception on their patients' anxiety. Before the radiographic examination, sAA activity was significantly higher for women (p ≤0.05). SAA activity significantly increased after the examination for men, patients of both sexes with no previous experience and for those who underwent CBCT (p≤0.05). Anxiety was reported to be observed in adult patients' speech and pediatric patients' behavior. Dental radiographic examinations may be a source of anxiety for women before the examination. After the examination, men, patients with no previous experience, and those subjected to CBCT present higher anxiety levels. Dentists perceive anxiety in adult and pediatric patients differently.


RESUMEN: Este estudio evaluó los niveles de ansiedad en los pacientes relacionados con exámenes radiográficos dentales, además de la percepción y el manejo de los dentistas. Se midió la actividad de la alfa-amilasa salival (sAA) de 47 pacientes antes y después de la exposición a exámenes radiográficos para evaluar su nivel de ansiedad y se comparó mediante la prueba de Wilcoxon. La prueba de Mann-Whitney evaluó la actividad de la sAA relacionada con el sexo, el tipo de examen y la experiencia previa con exámenes radiográficos. Se entrevistó a 50 dentistas utilizando un guión semiestructurado de preguntas abiertas sobre la percepción de la ansiedad de sus pacientes. Antes del examen radiográfico, la actividad de la sAA era significativamente mayor para las mujeres (p≤0.05). La actividad de SAA aumentó significativamente después del examen para los hombres, pacientes de ambos sexos sin experiencia previa y para los que se sometieron a CBCT (p≤0.05). Se informó que se observa ansiedad en el habla de los pacientes adultos y en el comportamiento de los pacientes pediátricos. Los exámenes radiográficos dentales pueden ser una fuente de ansiedad para las mujeres antes del examen. Tras la exploración, los hombres, los pacientes sin experiencia previa y los sometidos a CBCT presentan niveles de ansiedad más elevados. Los dentistas perciben la ansiedad en los pacientes adultos y pacientes pediátricos de manera diferente.

Introduction The side-to-end hypoglossal-facial anastomosis (HFA) technique is an excellent alternative technique to the classic end-terminal anastomosis, because itmay decrease the symptoms resulting from hypoglossal-nerve transection. Methods Patients with facial nerve palsy (House-Brackmann [HB] grade VI) requiring facial reconstruction from 2014 to 2017were retrospectively included in the study. Results In total, 12 cases were identified, with a mean follow-up of 3 years. The causes of facial paralysis were due to resection of posterior-fossa tumors and trauma. There was improvement in 91.6% of the patients (11/12) after the HFA. The rate of improvement according to the HB grade was as follows: HB III - 58.3%; HB IV - 16.6%; and HB II - 16.6%. The first signs of improvement were observed in the patients with the shortest time between the paralysis and the anastomosis surgery (3.5months versus 8.5 months; p » 0.011). The patients with HB II and III had a shorter time between the diagnosis and the anastomosis surgery (mean: 5.22 months), while the patients with HB IV and VI had a longer time of paresis (mean: 9.5 months; p » 0.099). We did not observe lingual atrophy or changes in swallowing. Discussion and Conclusion Hypoglossal-facial anastomosis with the terminolateral technique has good results and low morbidity in relation to tongue motility and swallowing problems. The HB grade and recovery appear to be better in patients operated on with a shorter paralysis time.

ABSTRACT: Panoramic radiography has clinical importance in several dental specialties. The objective of this study was to evaluate the influence of professionals' experience and patient characteristics (age group and dental absence) in the prevalence of errors and the number of retakes in the panoramic radiography.771 panoramic radiographs were evaluated and classified according to the group of patients (children, dentulous adults, total edentulous adults, and anterior edentulous adults) and professionals who performed the technique (novice and experienced). The association between frequency and type of error, group of patients, professionals' experience, and number of retakes was performed by the Chi-square and Fisher's exact test (a=0.05). There was no significant association between the frequency of errors or retakes of the radiograph s and the professionals' experience or patients' characteristics (p<0.05). The errors "turned head" and "tongue not against the palate," "chin tipped low", "chin not resting on the support," "patient movement during exposure," and "lead apron artifact" showed significantly higher frequency of retake (p<0.05). The most frequent type of error was due to the failure to position the tongue against the palate for all patients and both groups of professionals. The frequency of errors in the panoramic radiography occurs regardless of the professionals' experience, and age and dental absence of the patients. Error occurs mainly due to the lack of patient collaboration, and the examination is rarely repeated by the professionals.


RESUMEN: La radiografía panorámica es de importancia clínica en varias especialidades dentales. El objetivo de este estudio fue evaluar la experiencia de los profesionales y las características de los pacientes (grupo etario y ausencia dentaria) en la prevalencia de errores y el número de repeticiones en la radiografía panorámica.771 radiografías panorámicas fueron evaluadas y clasificadas según la grupo de pacientes (niños, adultos desdentados, adultos desdentados totales y adultos desdentados anteriores) y profesionales que realizaron la técnica (novatos y experimentados). La asociación entre frecuencia y tipo de error, grupo de pacientes, experiencia de los profesionales y número de repeticiones se realizó mediante la prueba de Chicuadrado y exacta de Fisher (a = 0,05). No hubo asociación significativa entre la frecuencia de errores o repetición de las radiografías y la experiencia de los profesionales o las características de los pacientes (p <0,05). Los errores "cabeza girada" y "lengua no contra el paladar", "mentón inclinado hacia abajo", "mentón no apoyado en el soporte", "movimiento del paciente durante la exposición" y "artefacto del delantal de plomo" mostraron una frecuencia de repetición significativamente mayor (p < 0,05). El error más frecuente se debió a la falta de posicionamiento de la lengua contra el paladar para todos los pacientes y en ambos grupos de profesionales. La frecuencia de errores en la radiografía panorámica ocurre independientemente de la experiencia de los profesionales, edad y ausencia dentaria del pacientes. El error se produce principalmente por la falta de colaboración del paciente, y los profesionales rara vez repiten el examen.

Abstract Objectives: to describe life quality of women with HPV and analyze the accuracy of anal visual inspection with acetic acid and lugol compared to high-resolution anoscopy in the detection of anal intraepithelial lesions. Methods: diagnostic evaluation study conducted with adult solid-organ transplant recipients and/or diagnosed with HIV and/or with a history of anogenital neoplasia, attended at the High Resolution Anoscopy outpatient clinic between April and December 2018. To calculate the accuracy parameters, 87 women participated: 44 with lesions and 43 without them. The 44 women with lesions were also subjected to a descriptive study to assess their life quality using the SF-36 questionnaire. Results: it was observed that almost half of the women in the study practiced vaginal, anal and oral intercourse without condom. History of HPV infection and anogenital neoplasia were the most prevailing with percentages of 81% and 72.7%, respectively. In five out of the eight domains of SF-36 they did not perform well in regard to life quality. Visual inspection with acetic acid and lugol presented sensitivity of 22.7% and specificity of 100%. Conclusion: unsatisfactory life quality was evidenced. Direct visual inspection with acetic acid and lugol should not be used in trials to spot intraepithelial HPV lesions, for it presents low sensitivity.


Resumo Objetivos: descrever a qualidade de vida das mulheres com HPVe analisar aacurácia da inspeção visual anal com ácido acético e lugol comparada à anuscopia de alta resolução para detecção de lesão intraepitelial anal. Métodos: estudo de avaliação diagnostica realizado com mulheres adultas transplantadas de órgãos sólidos e/ou diagnosticadas com HIV e/ou com antecedente de neoplasia anogenital, atendidas no ambulatório de AAR entre abril e dezembro de 2018. Para cálculo dos parâmetros de acurácia, participaram 87 mulheres: 44 com lesões e 43 sem lesões. Das 44 mulheres com lesão, também foi realizado um estudo descritivo para a avaliar a qualidade de vida utilizando-seo questionário SF-36. Resultados: Observou-se que quase metade das mulheres do estudo são adeptas ao intercurso vaginal, anal e oralesem preservativo. Os antecedentes de infecção pelo HPV e neoplasia anogenital foram os mais prevalentes com valores de 81% e 72, 7% respectivamente. Cinco, dos oito domínios do SF-36, não apresentaram boa performance em relação à qualidade de vida. A inspeção visual com ácido acético e lugol apresentou sensibilidade de 22,7%o e especificidade de 100,0%o. Conclusão: evidenciou-se qualidade de vida insatisfatória. A inspeção visual anal direta com ácido acético e lugol não deve ser usada na triagem de lesões intraepiteliais por HPV, pois apresenta baixa sensibilidade.

Objetivo: Conhecer como ocorre a comunicação de notícias difíceis por enfermeiros na atenção básica. Método: pesquisa exploratória, descritiva e de abordagem qualitativa com 15 Enfermeiros de 10 Unidades Básicas de Saúde no sul do país. A coleta de dados foi realizada através de entrevistas semiestruturadas. Os dados foram analisados conforme análise textual discursiva. Resultado: Foram geradas duas categorias: Comunicação como suporte as necessidades do cliente; Comunicação como elemento a suprir a demanda da atenção primária. Conclusão: os enfermeiros adotam alguns aspectos no momento da comunicação de notícia difícil, aspectos esses ancorados, por exemplo, através de empatia, compreensão da realidade do usuário, fala simplificada, privacidade, entre outros, sendo esses essenciais para uma comunicação adequada e humanizada que visa o conforto do usuário nesse momento da comunicação


Objective: To know how the communication of difficult news by nurses in primary care occurs. Method: exploratory, descriptive and qualitative approach with 15 Nurses from 10 Basic Health Units in the south of the country. Data collection was done through semi-structured interviews. Data were analyzed according to discursive textual analysis. ABSTRACT

Breast cancer is the most common malignancy in women worldwide. After mastectomy, many women wish to reconstruct the affected breast and immediate breast reconstruction has been proven oncologically safe compared to mastectomy only. In addition, indications for post-mastectomy radiotherapy (PMRT) sessions are becoming more frequent, due to their relationship with reduced mortality and locoregional recurrence. For this reason, many women who opted for immediate reconstruction underwent radiotherapy with implants or expanders. The objective of this study was to compare the outcome of patients with breast cancer undergoing adjuvant radiotherapy after breast reconstruction with an implant or expander with that of patients who did not need irradiation. A literature review was carried out on the CAPES Journal Portal. The studies are unanimous when it comes to the increased risk of complications between control groups and patients who have undergone PMRT. Reconstruction failure rates were lower and the esthetic results were better in surgeries with implants compared with tissue expanders. Autologous surgeries are apparently safe and should be considered in the context of PMRT. This review did not find sufficient scientific evidence to determine the best technique and the best period for radiotherapy in PMRT indications. The choice of the surgical technique and the time of radiotherapy must be at the discretion of the surgeon and multidisciplinary team of each service, always in a shared decision with the patient.

Resumo As alterações vasculares ocorrem frequentemente em região de cabeça e pescoço, sendo o hemangioma a mais comum. Paciente do sexo feminino, 61 anos, queixou-se de dor intensa em palato duro. Notou-se lesão arroxeada, de 1,5 cm, sensível à palpação e com histórico de hemorragia. A paciente era edêntula total, e a prótese total superior comprimia o local da lesão. Foi realizada a vitropressão, confirmando a origem vascular. A hipótese diagnóstica foi de hemangioma. Na primeira sessão, aplicou-se o laser vermelho (660 nm) em quatro pontos ao redor da lesão, sendo 0,5 J em cada ponto afim de se obter analgesia e iniciar o processo de reparo, além do reembasamento da prótese total superior. Na segunda sessão, foi feita aplicação de 2 mL de oleato de monoetanolamina 5%. Após 14 dias, observou-se regressão total da lesão. Os cirurgiões-dentistas devem estar aptos a reconhecer, diagnosticar e tratar as lesões vasculares em cavidade oral.


Abstract Vascular changes frequently involve the head and neck region and hemagioma is the most common. A 61-year-old female patient complained of severe pain in the hard palate. A purple lesion was found, measuring 1.5 cm, sensitive to palpation, and with a history of hemorrhage. The patient was fully edentulous and her upper denture compressed the lesion site. Diascopy confirmed the lesion's vascular origin. A diagnostic hypothesis of hemangioma was raised. In the first session, red laser light (660nm) was applied at 4 points around the lesion, with 0.5 J at each point, in order to obtain analgesia and trigger the repair process. The upper denture was also relined. In the second session, 2 mL of 5% monoethanolamine oleate was applied. After 14 days, total regression of the lesion was observed. Dental surgeons must be able to recognize, diagnose and treat vascular lesions in the oral cavity.
